ANIMAL BITE MANAGEMENT

 

 The Rabies is an acute viral disease caused by animal bite. Not only blood but from the saliva also rabies can be transmitted. The fatality rate is nearly 100%. Once the symptom is developed in rare cases patient is survived. There is no known effective treatment. So the animal bite management should be dealt with utmost care. 

Almost  all warm blooded animals including man can cause the disease. In almost 96% cases dogs are responsible for human rabies. Next is the cat (2%). 

Animal transmitting Rabies:

A. Domestic: Dogs and cats 

B. Peri-domestic:  Large ruminants (cows and buffaloes)  Small ruminants (sheep and goat),  Mammals  like pigs, donkeys, horses, camels etc. 

C.  Wild: Any wild animal Foxes and jackals, mongoose, leopard, bear etc. and even monkey. 

D.  Not reported: Bird, squirrel, bats, rodents 

Human-to-human transmission has never been confirmed other than organ transplantation. Organ transplanted from rabies –infected donors can transmit the infection to the organ recipient. Individuals with symptoms of unexplained encephalitis before death should, therefore be excluded as organ donors. Also, people who have been exposed closely to the secretions of a patient with rabies should be offered PEP as a precautionary measure.  

The observation period of 10 days is valid only for dogs and cats due to the fact that the incubation period of rabies is known and quite specific in dogs & cats, unlike in other animals. If the biting dog or cat had rabies virus in its saliva when it did the biting, research shows that it would die or show clinical signs of rabies within 10 days of bite.

How to manage the wound

 a. Wash the wound immediately (as early as possible) under running tap water for at least 15 minutes.

b. Use soap or detergent to wash the wound 

c. After thorough washing and drying the wound apply antiseptic e.g. Povidone iodine, etc. 

d. Don’t apply irritants viz. chillies, soil, oils, turmeric powder, lime, salt, plant juice etc.

e. Don’t touch the wound with bare hands. 

Wound washing must be performed even if the patient is late.

 Anti Rabies Vaccination

Fortunately, rabies is a preventable disease. PEP, which involves a series of shots and sometimes rabies immunoglobulin, can prevent the disease from developing if administered promptly after exposure.

Fever is not a disease. When a person’s body temperature increases above the normal range, we call the person is suffering from fever. The normal body temperature slightly differs from one to another. Normally the body temperature of a person is ranging between 37.1 and 38.3 C (98.6 to 100 F). Remedies: In most of the cases intake of plenty fluid, light diet and rest can help to reduce the temperature. Home remedies include turmeric, ginger, basil (tulsi), lemon-honey, coriander seed power (Dhaniya) and cumin-water (jeera). Paracetamol may help to lower the body temperature. Doctor will prescribe as per the body weight and age. At least six hours gap should be maintained between intake of medicine. For high fever sponging with cool water and placing wet cloth on forehead help to lower the temperature. For children cool water should not be used. Doctor’s attention is required. For adult also when the patient feels breathing trouble or loss of consciousness or high temperature (above 103 F) etc visit a doctor as soon as possible.

COVID-19 is a highly infectious disease and transmitted from person to person through droplet. Novel Coronavirus was first identified in Wuhan, Hubei Province of China in November 2019. Initially, WHO announced in social media that cluster of pneumonia cases with no death is happened in Wuhan. On 11th March 2020 WHO declared COVID-19 as a pandemic.

Prevention

Prevention is always better than cure. Since vaccine not yet developed, we should take sufficient precautions to avoid COVID-19 infections. Older adults, children and people with co-morbidities or pre-existing medical conditions like liver disease, heart diseases, asthma, HIV, Cancer, diabetes need extra precautions. One can be exposed to COVID-19 in high temperature also. Drinking alcohol also does not prevent or cure COVID-19.  

 How long Coronavirus will survive on surface, it mainly depends upon the materials. On plastic it can survive for longer period (72 hours). On other materials it can survive 4 hours or longer.

To prevent yourself from COVID-19  following precautionary measures can be taken.

1.      Maintaining at least one meter distance from people
2.      Frequent washing of hands with soaps  or using alcohol based sanitizers
3.      Avoid touching mouth, nose and eyes
4.      Covering mouth, nose and eyes
5.      Cleaning and disinfecting frequently touched surfaces.

Bronchitis

Swine Flu(H1N1): What is Swine Flue, Symptoms, Treatment

Recent outbreak of Swine Flu makes the world perturbed. The 2009 flu outbreak was fist noticed in Mexico. All of a sudden a large number of people infected with a common influenza-like Flu. Initially it was presumed that is normal seasonal influenza. But U.S labs confirmed that it is Swine Flu. Many countries including India advised their citizens not to visit foreign countries without essential reason.
What is Swine Flu:
WHO termed the new influenza virus as A(HINI). But it is widely used in the mass media as Swine Flu. Earlier Swine Flu virus and human flu virus spreads differently. In the past Swine Flu affected people had a direct contact with pigs. But in this case though it is originated in pigs but can spread from person to person. It is not necessary to have a contact with pigs.

Symptom of Swine Flu:
Symptoms of Swine Flu are similar to that of common Flu. Fever, chills, headache, body ache, sore throat, fatigue ness are the symptoms of Swine Flu. Besides these a Swine Flue affected person might have the symptoms of diarrhea, vomiting, rapid breathing and excessive sleeping.

How Swine Flu spreads:
It spreads like common flu. It can spread through air from cough and sneezes. One may be infected if he touches any thing with Swine Flu and after that touch his mouth, even nose or eye.

3. Covering mouth and nose at the time of coughs and sneezes.

Treatment of Swine Flu:
Treatment should be started as early as possible. Generally two medicines are prescribed for Swine Flue- oseltamivir and zanamivir. Duration of treatment upto one week. Antiviral can be used to combat it just like recommended for seasonal influenza. however there is no vaccine for HINI.

Pregnancy and transmission of HIV

It is utmost importance that a mother should be aware of the possibility of HIV transmission to her baby. She should be undergone test before or on the onset of pregnancy so that sufficient precautions can be taken for prevention of perinatal HIV infection. Without proper treatment up to 30% of babies infected with HIV may born to the women having HIV positive. Moreover after delivery up to 20% babies may be infected with HIV through breast-feeding.

Transmission of HIV
HIV or Human Immunodeficiency Virus causes AIDS or Acquired Immune Deficiency Syndrome. Although HIV is mostly passed from one person to another through sexual contact, but it also spreads through body fluids like blood, semen, vaginal fluid, breast milk. HIV is usually transmitted through sharing needles, unprotected anal, vaginal and sometimes oral sex and from mother to infant before or during delivery or while breastfeeding.

How HIV pass on to baby from mother
Baby may get infected with HIV in the womb, in the vaginal passage and through breast milk. Transmission of HIV to baby may take place during pregnancy, during labor and during delivery or after delivery through breast feeding.
During pregnancy, the mother’s antibodies against HIV are passed on to her baby. So some babies of women with HIV may test positive for HIV antibodies at first. This does not necessarily imply that the baby is infected. The baby’s immune system will start making antibodies of their own only between 6 and 18 months of age. If the baby is infected with HIV, he or she will still lose the mother’s antibodies, but the baby will start to make antibodies to HIV. The baby will test positive and continue to test positive for HIV.

Tests
The CD4 test gives the immunological status while the viral load test gives information about the severity of infection. These results guide the clinician when to initiate therapy and regimens to be adopted. Other advanced tests like PCR and viral culture can also be used to check babies for HIV infection depending on availability of tests.

Prevention
Voluntary testing and counseling is required for preventing HIV infection among parents. HIV positive women should avoid unwanted pregnancies and access to antiretroviral therapy, safe delivery practices, and the widespread availability and safe use of breast-milk substitutes are the effective ways of prevention of mother to child transmission. Since HIV is found in breast milk, so there is a chance to pass HIV to the baby. Boiled milk can be fed to baby to stop the transmissions of HIV as heat can destroy the HIV. Shorter period of delivery time can decrease the chance of transmission of HIV. Mothers with a high viral load might reduce their risk if they give birth their baby by cesarean section. This helps in protecting the child from direct contact with her blood and other bodily fluids. Antiretroviral can minimize the possibility HIV transmission to baby. ARVs are assumed to be safer for the baby but long-term effects are as yet unknown. The earlier HIV is diagnosed during, before or after pregnancy, the better are the chances of success. The most important time for an HIV positive pregnant woman to take ARVs is during labor. Transmission rates are only 1% to 2% if the mother takes combination antiretroviral therapy
